Welcome to the T Off Your Health family. We look forward to providing you with personalized and comprehensive healthcare focusing on wellness and prevention. Our Physicians, Nurse Practitioners, Medical Assistants and Office Staff work closely together as a team to support your overall care as a patient here at T Off.

Once contacted and scheduled for an appointment you will receive a welcome e-mail containing your new patient information that you must fill out and bring with you to your appointment. Please bring your ID and insurance card with you as well. Also remember, if you will be doing bloodwork make sure to fast for 8-10 hours and drink lots of water!
